Biology & mechanism

A different engineering solution to the same half-life problem. Rather than acylation and albumin binding, dulaglutide covalently fuses two GLP-1 analog molecules to an IgG4 Fc fragment. The resulting protein is far too large for renal clearance and is recycled by the neonatal Fc receptor, giving a weekly dosing interval. Receptor pharmacology is standard GLP-1.

What the research actually shows

The AWARD programme established glycaemic efficacy in type 2 diabetes, and REWIND reported cardiovascular outcome benefit in a population with a substantial primary-prevention component. A mature, well-evidenced agent.

Regulatory status

FDA-approved for type 2 diabetes and cardiovascular risk reduction. Prescription-only. Not approved for weight management, unlike semaglutide and tirzepatide.

Safety signals

Boxed warning for rodent thyroid C-cell tumours; MTC/MEN2 contraindication. Gastrointestinal effects. Physician supervision required.
